The following are the needed eligibility requirements to avail our home health care services:
- Must be MEDICARE Certified
- Must be HOMEBOUND – this means that the patient must experience a taxing effort to leave the home, and absence from the home should be for short periods only and infrequent
- The care that the patient requires must be intermittent – this means that the care the patient receives must be more than one visit and a reasonable level of rehabilitation can be expected with treatment.
